Ictal Pure Word Deafness with Auditory Hallucination
نویسنده
چکیده
Pure word deafness is a disorder of auditory verbal comprehension without dysfunction of speaking, reading, or writing. This rare symptom arises from bilateral lesions in the temporal lobes or disconnection between the primary auditory cortex and Wernicke’s area. I present a right-handed 33-year-old woman who presented with pure word deafness and complex auditory hallucination with intermittent amnesia. She had no generalized convulsion. There was no intracranial lesion on plain or gadoliniumenhanced magnetic resonance imaging. Electroencephalogram revealed theta waves in the left temporal-occipital region followed by high-amplitude slow wave burst during hyperventilation. Carbamazepine completely ameliorated the pure word deafness and auditory hallucination. This is the first report of pure word deafness resulting from primary complex partial seizure. The epileptic focus might lie along the auditory word processing pathway between the left auditory cortex and Wernicke’s area.

Ictal deafness—a report of three cases
Though negative phenomena like motor inhibition, blindness or aphasia are described as an ictal manifestation of seizures, ictal deafness has not been reported so far. We observed transient ictal deafness in three cases of partial seizures. One of them had seizure spread to the temporal lobe to produce a complex partial seizure. Two of them have CT-detected lesions in the left temporal/parietal...
متن کاملWord deafness: one hundred years later.
Since its original description the diagnosis of word deafness has been greatly expanded. Confusion has arisen with regard to the usage of the related terms pure word deafness, auditory agnosia, and cortical deafness. Three new cases of word deafness are presented including one case with CT and necropsy correlation. These cases are compared with 34 previously reported cases of various cortical a...
متن کاملWord deafness in Wernicke's aphasia.
Three patients with otherwise typical Wernicke's aphasia showed consistent superiority of visual over auditory comprehension. The precedents for and anatomical basis of a selective auditory deficit in Wernicke's aphasia are discussed, including the relationship to pure word deafness. One implication of spared visual language function may be the use of gesture in language therapy for such patients.
متن کاملReversible pure word deafness due to inferior colliculi compression by a pineal germinoma in a young adult.
We report a clinical case of a 19-year-old male patient who developed pure word deafness due to the local compressive effect of a pineal germinoma on the inferior colliculi of the quadrigeminal plate. After percutaneous radiation therapy the size of the tumor decreased significantly, while audiometry demonstrated a complete regression of the auditory deficit. Since pure word deafness is commonl...
متن کاملCortical deafness in multiple sclerosis.
Cortical deafness in a patient with multiple sclerosis is reported. Complete recovery from total deafness was seen following stages of auditory agnosia and pure word deafness. The otological and neurophysiological studies suggested lesions in subcortical white matter. This report stresses the rarity of the condition, its subcortical origin and good prognosis.
